Creditor List You MUST list all people you owe money to and complete all fields including collection agency, collateral, and co-signer. You MUST list the following even if you want to keep the secured property and/or the debt is non-dischargeable:

* Credit cards, medical bills, personal loans, overdue bills.
* Bank income taxes owed to IRS, State, City.
* Child support, alimony, other domestic support.
* Student loans.
* Negligence claims, claims for intentional harm, claims for theft or fraud.
* Overpayment of benefits.
* Parking tickets, traffic tickets.
* Debts to family members or any other claims.
* Currently active lease on vehicles, real estate, personal property.

Budget Form – List your household size, employment information, income information, and monthly expenses.

CMA/Appraisal Request Form – Use this to request a market valuation from Peerless Appraisal. If you are doing a Chapter 13 to “strip” your second mortgage, you MUST have a full appraisal. All others can get a CMA.

IRS Transcript Request – We need your tax transcripts to file your case.

United States Trustee’s Bankruptcy Information Sheet – You must read this document in full and understand it so that your Meeting of the Creditors may be conducted. If you still haven’t filed yet, please review as it will answer questions for you.
